THE POSITION:
  • Seeking a full-time AdvanceClinical Pharmacist responsible for for providing all levels of pharmacyclinical services. This includes monitoring patient medication regimens andproviding appropriate interventions as well as supporting all aspects ofpharmacy operations.
  • Interpret orders andtranscribes or verifies them accurately and efficiently into the pharmacycomputer system
  •  Accurately compound and dispensepharmaceuticals
  • Complies with facility policyand procedures and regulatory requirements
  • Monitor drug therapy regimensfor contraindications, drug-drug interactions, drug-food interactions,allergies and appropriateness of drug and dose
  •  Provide clinical consultation andclarification to practitioners. Suggests appropriate, cost-effectivetherapeutic alternatives to medical staff as needed.
  • Sustain the hospitalformulary, minimizing non-formulary procurements, utilizing therapeuticinterchanges and promotes rational drug therapy selection
  •  Attend and actively participate in dailymultidisciplinary and bedside rounds
  •  Support the hospital formulary decisionprocess by working with medical staff to promote rational drug therapy and makeimprovements identified by drug utilization evaluations
  •  Provide accurate and timely medicationinformation to healthcare staff and patients
  • Actively participate inpharmacy clinical services such as drug consults, nutrition support,pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renaldosing and other programs as defined by Pharmacy Leadership.
  •  Participate in the antimicrobial managementand other clinical programs
  •  Document all clinical activities andinterventions accurately and completely. Responsible for mentoring andeducation as well as ongoing analysis of interventions to insure accurate andappropriate documentation and provide summary reports (if assigned)
  •  Support and participate in the goals of theorganization’s quality improvement, risk management and patient safety programto include but not limited to National Patient Safety Goals, Core Measures andHCAHPS medication related initiatives.
  •  Complete notification reports regardingmedication errors and adverse drug reactions
  • Participate in the datacollection of drug utilization evaluations, adverse drug reactions, andpharmacy quality initiatives
  •  Lead the training and mentorship ofpharmacists, pharmacy students, residents, technicians, nurses and physicians.

EDUCATION
  • B.S. orPharmD from an ACPE-accredited School of Pharmacy, a Doctor of Pharmacy from anACPE-accredited college of pharmacy is preferred
EXPERIENCE
  • At leastone of the following:
  • Minimumof 1 year of hospital clinical pharmacy experience or at least 2 years ofgeneral hospital pharmacy experience required; preferably within area of focus
  • Completionof hospital PGY1/ pharmacy general practice residency or fellowship
  •  Specialty BCPS certification
  • Othercertification (e.g. FCCP, FASHP)
CERTIFICATE/LICENSE
  • Activepharmacist license in good standing with the appropriate State Board ofPharmacy 
PHYSICALDEMANDS/WORKING CONDITIONS Requires prolonged sitting, some bending, stooping andstretching. Requires eye-hand coordination and manual dexterity sufficient tooperate a keyboard, photocopier, telephone, calculator and other officeequipment. Requires normal range of hearing and eyesight to record, prepare andcommunicate appropriate reports. Requires lifting papers or boxes up to 50pounds occasionally. May be exposed to toxic/caustic/chemicals/detergents. Workis performed in a hospital (pharmacy, nursing units, etc.) Work may bestressful at times. Contact may involve dealing with angry or upset people.Staff must remain flexible and available to provide staffing assistance forany/all disaster or emergency situations.
