GENERAL SUMMARY OF DUTIES: The Facility Medication Safety Pharmacist is responsible for the medication safety and quality activities at the facility; duties include but are not limited to: standardization of medication management practices, ensure regulatory compliance, implementing national quality agenda related to medication management, development of medication safety programs, and staff development/education. Provides support for all medication management and quality initiatives at the facility and those impacted by centralized order entry.

DUTIES (included but not limited to): Develops and manages the facility Medication Safety Improvement Plan  Works with multidisciplinary groups at the facility level to ensure key quality initiatives are in alignment with organizational priorities  Develops facility pharmacy quality plan in conjunction with division leadership and clinical pharmacy group to ensure adequate budgetary support for medication safety programs  Serves as an internal consultant on medication/patient safety and quality standards related to medication management (ie. TJC, CMS, NQSF, IHI and FDA standards) and ensure TJC ongoing readiness  Identifies and facilitates medication safety improvement projects  Lead the facility medication safety committee, agenda items to include, but not limited to: ISMP quarterly review, policy review, tracers, survey results, MUE action items, RCA, FMEA, pump safety, and Med Watch, medication error review  Conduct gap analysis for specific performance improvement procesess, as needed  Manages data reporting and information related to medication safety, including medication events and ADRs.  Reviews and trends reported medication occurrences to identify and resolve potential system vulnerabilities.  Supports Pharmacy Director for the execution of facility and division pharmacy performance improvement plan (Quality KPI) at the facility.  Supports Pharmacy Director for the ongoing assessment, measurement and improvement of operations.  Leads facility efforts for the appropriate and safe use of pharmacy technology including IV pumps, Order Entry Systems and Pharmacy Automation  Participates in education, research and other professional activities including precepting pharmacy students and/or residents  Assists Pharmacy Administrative team for the pharmacy staff development/competency, training, and continuing education processes in collaboration with the Division Director of Medication Safety  Contributes to a culture of safety. Models optimal behavior and encourages others to participate in safety initiatives 

Job Requirements


Pharm.D. or Master’s Degree required  Residency Training or BCPS certification preferred


3-5 years experience as a clinician in a hospital setting or completion of Medication Safety Residency


Experience in Patient Safety Initiatives desirable Presentation, research and publishing experience desirable
