THE POSITION:
  • Seeking a full-time PharmacyClinical Manager responsible fordesigning, implementing and managing the pharmacy clinical programs. Thisincludes formulary management, policy and procedure development and educationacross the healthcare spectrum. The Clinical Manager shall also assist inmanaging the pharmacy when the Pharmacy Director or designee is unavailable.
  • Interpret orders and transcribes or verifies them accurately andefficiently into the pharmacy computer system
  • Accurately compound and dispense pharmaceuticals
  • Complies with facility policy and procedures and regulatoryrequirements
  • Monitor drug therapy regimens for contraindications, drug-druginteractions, drug-food interactions, allergies and appropriateness of drug anddose
  • Provide clinical consultation and clarification to practitioners.Suggests appropriate, cost-effective therapeutic alternatives to medical staffas needed
  • Sustain the hospital formulary, minimizing nonformularyprocurements, utilizing therapeutic interchanges and promotes rational drugtherapy selection
  • Support participation in daily multidisciplinary and bedsiderounds. (may need to attend)
  • Identify, design and analyze drug utilization reviews to promoterational drug therapy and determines action plan based on results
  • Provide accurate and timely medication information to healthcarestaff and patients
  • Develop, manage and mentor pharmacy clinical programs such as drugconsults, nutrition support, pharmacokinetics, anticoagulant monitoring, painmanagement, IV to PO, renal dosing and other programs as determined appropriatebased on patient population and literature guidelines.
  • Develop, manage and mentor antimicrobial management program (AMP)
  • Document all clinical activities and interventions accurately andcompletely. Responsible for mentoring and education as well as ongoing analysisof interventions to insure accurate and appropriate documentation and providessummary reports
  • Support and participate in the goals of the organization’s qualityimprovement, risk management and patient safety program to include but notlimited to National Patient Safety Goals, Core Measures and HCAHPS medicationrelated initiatives
  • Responsible for design, analysis and reporting of adverse drugreactions, medication errors and pharmacy quality initiatives. Mentors othersin their roles for these programs
  • Develop and actively participate in training and mentorshipprograms for pharmacists, pharmacy students, residents, technicians, nurses andphysicians Responsible for assessment of participants in these programs
  • Develop and document staff competency on an ongoing basis anddevelop improvement plans when deficiencies in competencies are identified.

EDUCATION
  • B.S. or Pharm D. from an ACPE-accredited School of Pharmacy, aDoctor of Pharmacy from an ACPE-accredited college of pharmacy is preferred
EXPERIENCE
  • Minimum of 1 yearas a Pharmacy Clinical/Operations Specialist or one of the following:
  • At least 5 yearsof hospital pharmacy experience required; preferably within area of focus
  • Completion ofhospital PGY2/ pharmacy specialty residency or fellowship
  • Specialty BCPScertification
  • Othercertification (e.g. FCCP, FASHP)
  • Hospital PharmacyManagement experience is preferred
CERTIFICATE/LICENSE
  • ACLS certification is preferred
  • Active Pharmacist license in good standingwith the appropriate State Board of Pharmacy
  • Active Pharmacy Consultant license, orequivalent as per State requirements for a Pharmacy Director/Pharmacist inCharge, if facility pharmacy does not have a Pharmacy Operations Managerposition available.

PHYSICALDEMANDS/WORKING CONDITIONS Requires prolonged sitting, some bending, stooping andstretching. Requires eye-hand coordination and manual dexterity sufficient tooperate a keyboard, photocopier, telephone, calculator and other officeequipment. Requires normal range of hearing and eyesight to record, prepare andcommunicate appropriate reports. Requires lifting papers or boxes up to 50pounds occasionally. May be exposed to toxic/caustic/chemicals/detergents. Workis performed in a hospital (pharmacy, nursing units, etc.) Work may bestressful at times. Contact may involve dealing with angry or upset people.Staff must remain flexible and available to provide staffing assistance forany/all disaster or emergency situations.
