Job Description

The GeneralClinical Pharmacist is responsible for providing general clinical and pharmacyservices. This includes supervision of technical staff.
Supervisor – PharmacyDirector, Pharmacy Operations Manager /Pharmacy Clinical Manager
 
Supervises – Pharmacy Technicians
 
KeyInteractions – Pharmacists,Pharmacy technicians, Nurses, Physicians, Pharmacy Students, Pharmacy Internsand other Facility Staff

Duties (included but notlimited to):
 

  • Interpret ordersand transcribes or verifies them accurately and efficiently into the pharmacycomputer system
  • Accuratelycompound and dispense pharmaceuticals
  • Complies withfacility policy and procedures and regulatory requirements
  • Monitor drugtherapy regimens for contraindications, drug-drug interactions, drug-foodinteractions, allergies and appropriateness of drug and dose
  • Suggestsappropriate, cost-effective therapeutic medication alternatives to medicalstaff
  • Sustain thehospital formulary, minimizing non formulary procurements, utilizingtherapeutic interchanges and promotes rational drug therapy selection
  • Provide accurateand timely medication information to healthcare staff and patients
  • Assist withclinical services such as drug consults, nutrition support, pharmacokinetics,anticoagulant monitoring, pain management, IV to PO, renal dosing and otherprograms as defined by Pharmacy Leadership.
  • Assist with theantimicrobial management and other clinical programs
  • Document allclinical activities and interventions accurately and completely
  • Support andparticipate in the goals of the organization’s quality improvement, riskmanagement and patient safety program to include but not limited to NationalPatient Safety Goals, Core Measures and HCAHPS medication related initiatives.
  • Completenotification reports regarding medication errors and adverse drug reactions
  • Participate inthe training of new pharmacists, pharmacy students, and technicians
  • Maintainprofessional competency sufficient to meet current and future needs of thefacility
  • Has knowledge ofthe patient population served and apply that knowledge in the care of thosepatients
  • Provide basictechnical support for automated dispensing machines and other pharmacy computersystems
  • Effectivelysupervise pharmacy operation activities and provides support to the wholepharmacy team
  • Compliant withfacility pharmacy schedule to work physically at patient care areas to interactwith medical staff, nurses and other members of the healthcare team.
  • Effectively andproactively intervene in conflicts between pharmacy and medical staff regardingdrug therapy
  • issueresolutions.
  • Train andparticipate with Disaster Team drills and real-event response as assigned
  • Trainand participate with Code Teams as assigned. Obtain and maintain BLS-ACLS orPALS current certification as appropriate.
  • Maintain appropriate controlled substance records
  • Complywith all regulatory standards
  • Prioritizework and lead others to comply with facility medication turn-around time policyand procedure. Identify areas for improved efficiency
  • Superviseand assist pharmacy technicians and other support staff effectively
  • Actas supervisor in charge as assigned
  • Participatein the continuous quality improvement of pharmacy operations(operational/clinical)
  • Provideinput for improving pharmacy computer system programs relating to pharmacy,nursing and physician programs
  • Performother duties as assigned

EDUCATION

  • B.S. or PharmDfrom an ACPE-accredited School of Pharmacy

 
EXPERIENCE

  • Minimum of oneyear of hospital experience is preferred

 
CERTIFICATE/LICENSE

  • Active pharmacistlicense in good standing with the appropriate State Board of Pharmacy 


PHYSICAL DEMANDS/WORKING CONDITIONS – Requires prolonged sitting, some bending, stooping andstretching. Requires eye-hand coordination and manual dexterity sufficient tooperate a keyboard, photocopier, telephone, calculator and other officeequipment. Requires normal range of hearing and eyesight to record, prepare andcommunicate appropriate reports. May be exposed totoxic/caustic/chemicals/detergents. Work is an office or pharmacy environment.Work may be stressful at times. Contact may involve dealing with angry or upsetpeople. Staff must remain flexible and available to provide staffing assistancefor any/all disaster or emergency situations.
 
 

OSHA CATEGORY – The normal work routine involves no exposure to blood, body fluids, ortissues (although situations can be imagined or hypothesized under whichanyone, anywhere, might encounter potential exposure to body fluids). Personswho perform these duties are not called upon as part of their employment toperform or assist in emergency care or first aid, or to be potentially exposedin some other way.
