Pharmacist, Centralized Order Entry  
Part-time / Work From Home / Nashville area
 
 
The COE Pharmacist is responsible formonitoring, evaluating and making recommendations to assure the appropriate andeffective use of pharmaceuticals for patient care. The COE pharmacist shall usethe medication process to assess, plan, intervene when necessary, implement andevaluate patient care in compliance with client hospital policies, protocolsand procedures.
 
 
ESSENTIAL DUTIES:
• Demonstrate knowledge and mastery ofrequired job functions, internal communications and operating systems
• Accurately enter orders in thecomputer in a timely manner
• Support and assist in meetingdepartmental goals for order entry and clinical programs
• Ensure facility procedures arefollowed when non-formulary drugs, target drugs, restricted drugs and investigational drugs are ordered
• Initiate change in assignments basedon workload and resources available
• Demonstrate proficiency in computerorder entry and related computer functions
• Screen for drug interactions,allergies, order duplications, appropriate diagnosis, renal and liver function prior to order entry
• Assure facility queues are addressedin a timely manner
• Investigate and report adverse drugevents and medication incidents
• Facilitate dissemination of druginformation in a timely manner to achieve high quality, cost effective, positive therapeutic outcomes whenconsulted
• Review and interpret all physicianorders received, using the patient profile
• Monitor for incompatibilities,concentration and rate on intravenous drugs
• Participate in the maintenance ofcontinuous quality improvement program
• Assist in the development and reviewof COE protocols
• Follow CSC pharmacy policies andprocedures
 
 
 

Job Requirements

EXPERIENCE / EDUCATION / LICENSE
 
• One (1) year hospital experience highlypreferred
• Meditech preferred
• Bachelor’s degree from an accreditedcollege of pharmacy, Pharm.D. preferred
• Appropriate State Pharmacy License(s)
