Job Description

The PharmacyClinical/Operations Specialist is responsible for participating and leadingpharmacy clinical or operational programs. This includes monitoring patientmedication regimens and providing appropriate interventions. The ClinicalSpecialist shall also assist in mentoring pharmacy staff and students andassist in the evaluation and implementation of clinical/operational programs.
Duties (included but notlimited to):
  • Interpret ordersand transcribes or verifies them accurately and efficiently into the pharmacycomputer system.
  • Accuratelycompound and dispense pharmaceuticals.
  • Complies withfacility policy and procedures and regulatory requirements.
  • Monitor drugtherapy regimens for contraindications, drug-drug interactions, drug-foodinteractions, allergies and appropriateness of drug and dose.
  • Provide clinicalconsultation and clarification to practitioners. Suggests appropriate,cost-effective therapeutic alternatives to medical staff as needed.
  • Sustain thehospital formulary, minimizing nonformulary procurements, utilizing therapeuticinterchanges and promotes rational drug therapy selection.
  • Attend andactively participate in daily multidisciplinary and bedside rounds.
  • Complete drugutilization reviews to promote rational drug therapy and implement improvementsidentified.
  • Provide accurateand timely medication information to healthcare staff and patients.
  • Lead and developpharmacy clinical services such as drug consults, nutrition support,pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renaldosing and other programs as defined by Pharmacy Leadership. Develop, manageand mentor antimicrobial management program (AMP).
  • Document allclinical activities and interventions accurately and completely. Responsiblefor mentoring and education as well as ongoing analysis of interventions toinsure accurate and appropriate documentation and provide summary reports.
  • Support andparticipate in the goals of the organization’s quality improvement, riskmanagement and patient safety program to include but not limited to NationalPatient Safety Goals, Core Measures and HCAHPS medication related initiatives.
  • Participate inthe design, collection, and reporting of adverse drug reactions, drugutilization reviews, medication errors and pharmacy quality initiatives.
  • Lead the trainingand mentorship of pharmacists, pharmacy students, residents, technicians,nurses and physicians. Develop and document staff competency on an ongoingbasis and develops improvement plans when deficiencies in competencies areidentified
  • Lead inpreceptorship of pharmacy students and/or residents.
  • Maintainprofessional competency sufficient to meet current and future needs of thefacility.
  • Provide anddemonstrate expertise in a specialized area of pharmacy practice (operationaland/or clinical).
  • Providebasic technical support for automated dispensing machines and other pharmacycomputer systems.
  • Effectivelysupervise pharmacy operation activities and provides support to the wholepharmacy team.
  • Compliantwith facility pharmacy schedule to work physically at patient care areas tointeract with medical staff, nurses and other members of the healthcare team.
  • Effectivelyand proactively intervene in conflicts between pharmacy and medical staffregarding drug therapy issue resolutions.
  • Train andparticipate with Disaster Team drills and real-event response as assigned.
  • Train andparticipate with Code Teams as assigned. Obtain and maintain BLS-ACLS or PALScurrent certification as appropriate.
  • Maintainappropriate controlled substance records. Work with the Pharmacy Director andappropriate manager to identify and develop processes to meet or exceed allcontrolled substance diversion prevention and monitoring standards. Participatein controlled substance reviews and audits.
  • Comply withall regulatory standards. Work with the Pharmacy Director to identify anddevelop processes to meet or exceed all regulatory standards. Participate inquality and regulatory reviews and audits.
  • Prioritizework and lead others to comply with facility medication turn-around time policyand procedure. Identify areas for improved efficiency. Assist in implementationof process changes.
  • Superviseand assist pharmacy technicians and other support staff effectively.
  • Participatein establishing pharmacy goals and objectives and assists in theimplementation.
  • Participatewith various nursing and medical staff committees as necessary.
  • Assist withstaff performance evaluations.
  • Act asmanager in charge as assigned.
  • Providereview and guidance on physician order sets and other evidenced basedmedication use guidelines.
  • Assist withthe preparation of the pharmacy newsletter and/or other forms of written andelectronic communication.
  • Perform drugclass reviews and drug monographs.
  • Participatein all aspects of the drug formulary management process.
  • Assist withthe development, implementation, and monitoring of pharmacy operationsinitiatives (operational/clinical).
  • Participatein the development and review of pharmacy policies and procedures.
  • Assist inthe design and update of pharmacy computer system programs relating topharmacy, nursing and physician programs.
  • Assist inthe maintenance of pharmacy dictionaries including adding new drugs to the drugdictionary and Gold Standard formulary Charge Master maintenance.
  • Performother duties as assigned.

  • B.S.or PharmD from an ACPE-accredited School of Pharmacy, a Doctor of Pharmacy froman ACPE-accredited college of pharmacy is preferred
At least oneof the following:
  • Minimumof 1 year experience as Advanced ClinicalPharmacist or at least 5 years of general hospital pharmacy experiencerequired; preferably within area of focus.
  • Completionof hospital PGY2/ pharmacy general practice residency or fellowship.
  • SpecialtyBCPS certification.
  • Othercertification (e.g. FCCP, FASHP).
  • Activepharmacist license in good standing with the appropriate State Board ofPharmacy
  • ACLScertification may be required for critical care/ED specialists
PHYSICALDEMANDS/WORKING CONDITIONS – Requires prolonged sitting, some bending, stooping and stretching.Requires eye-hand coordination and manual dexterity sufficient to operate akeyboard, photocopier, telephone, calculator and other office equipment.Requires normal range of hearing and eyesight to record, prepare andcommunicate appropriate reports. Requires lifting papers or boxes up to 50pounds occasionally. May be exposed to toxic/caustic/chemicals/detergents. Workis an office or pharmacy environment. Work may be stressful at times. Contactmay involve dealing with angry or upset people. Staff must remain flexible andavailable to provide staffing assistance for any/all disaster or emergencysituations.
OSHACATEGORY – Thenormal work routine involves no exposure to blood, body fluids, or tissues(although situations can be imagined or hypothesized under which anyone,anywhere, might encounter potential exposure to body fluids). Persons whoperform these duties are not called upon as part of their employment to performor assist in emergency care or first aid, or to be potentially exposed in someother way.
